Instructions for the organization
- Fill in the bracketed fields, then give the form to the person having parental authority or the tutor of each player under 14. For a player aged 14 to 17, have the player sign (and, if your policy requires it, a parent).
- Keep signed forms in your records (UFStats does not receive them and does not need them).
- In UFStats, team roster: mark each player Consent obtained or Consent refused. Without consent, the player's individual statistics are not displayed in youth mode.
- A withdrawal of consent must be recorded in UFStats without delay.

What we will do
Our organization films some ultimate matches and practices. Videos are uploaded to UFStats, a Québec video analysis service, which computes game statistics (passes, catches, points, movement) and links them to video sequences. These statistics are used for coaching and player development.
- Who sees the videos and statistics: only coaches, players and parents of the team who have access. No public sharing for a youth team.
- Information about your child in UFStats: first name (or display name), jersey number and computed statistics. No date of birth, address or facial recognition.
- Retention: the original video is deleted no later than 30 days after analysis; the viewing copy is kept according to the organization's plan (at most 2 years); statistics, as long as the organization's account exists.
- Model training: images of a youth team are never used to train UFStats models.
- Hosting: some UFStats providers are located outside Québec (in Canada and the United States); see the UFStats privacy policy (ufstats.tech/en/privacy).
Your rights
You may withdraw this consent at any time by notifying [contact person], and request access to your child's information, its correction or the removal of their individual data (their individual statistics are then deleted). Refusing or withdrawing consent does not prevent your child from playing: they will not appear in individual statistics.
